Yes, teens, you will need to complete ONE LAST PERMISSION FORM! ( • All teen team leaders are responsible for bringing the designated permission form to the first formation date. It covers you for all three dates, and you only need to submit one. You will be mailed one, or you can get it here ( PDF). Another form will cover you for the TAPT weekend. It will be given to you at the formation mtgs.

plans have changed and you are unable to attend these sessions, you must
inform Kevin as soon as possible so we can find a replacement for you!
Remember, there were others that were turned down and who would LOVE to take
your place! Do NOT take this responsibility lightly. If you cannot attend a
